A Study on Functional Outcome of Anterior vs. Posterior Circulation Stroke treated with Intravenous Thrombolysis

ABSTRACT
Background: Intravenous thrombolysis (IVT) is an approved treatment of acute ischemic stroke in anterior (ACS) and posterior (PCS) cerebral circulation.
Aims: The aim of this study was to assess the functional outcome of intravenous thrombolysis in anterior versus posterior circulation stroke patients.
Materials and Methods: The study design was prospective observational cohort study. Total sampling was 41 included in this study. The study was conducted at the Department of Neurology, Chalmeda Anand Rao Institute of Medical Sciences, Karimnagar. Patients were enrolled after a formal written consent and were subjected to a full neurological assessment including history and examination.
Results: In our study, 73% in anterior circulation stroke and 81% in Posterior circulation stroke. There was No significant difference between two groups seen. Mean NIHSS Score is 10.7 in anterior circulation stroke and 8.6 in posterior circulation stroke with significant difference (p< 0.008 ) between two groups.
Conclusion: Our study shows that frequency of stroke cases who were thrombolysed in ACS was 73% PCS was 27%. Patients with PCS had significantly lower baseline NIHSS scores (8.6%) as compared with those with ACS (10.7%) in the present study which is statistically significant. The functional outcome of the cases at 90 days was found to be better in PCS compared to ACS but was not statistically significant.
